Patients

Improving our health care system means increasing access to providers who deliver high-quality care, better-coordinating care among providers, and managing the rising cost of health care. That’s what we do, and that’s why we’re here.

We want a healthier you.

Adirondacks ACOs goal is to improve the health care system. This means increasing access for our communities to providers who deliver high-quality care, better-coordinating care between different providers, and assisting in managing the rising cost of health care. Adirondacks ACO works to help providers identify patients whose health may need improvement by providing data-centric tools to ensure providers can find the resources they need. All we want is a healthier you at the end of the day. That’s what we do, and that’s why we’re here.
