What We Do

Improving our health care system means increasing access to providers who deliver high-quality care, better-coordinating care among providers, and managing the rising cost of health care. That’s what we do, and that’s why we’re here. 

Adirondacks ACO is an Accountable Care Organization (or ACO) made up of groups of doctors, hospitals, and other health care providers, who all work together to deliver high-quality, coordinated care. By ensuring that the best quality care is provided at the right time, in the right place, an ACO can both improve health for patients, and reduce costs by creating a more efficient system. 

A well-functioning ACO can help ensure that important details of a patient’s complex medical condition don’t fall through the cracks among multiple providers, and it can prevent families from seeing higher and higher bills from their insurance providers every year. The ACO empowers patients like you to take control of your health by prioritizing wellness and prevention in addition to disease management. The goal is for patients to be proactive participants in their care, so you and your loved ones can be as healthy as possible. 

ADKs ACO currently includes more than 800 participating doctors, clinicians, and other health care professionals throughout Clinton, Essex, Franklin, Hamilton, Warren, Washington, and northern Saratoga counties of New York. We’re a big team, and we’re on your side!

Our Insurance Plan

Adirondacks ACO serves as the provider network for government-sponsored and commercial insurance plans called “payers” that are available to New York residents. By partnering with an ACO like us, government and commercial payers can offer affordable insurance products where the provider network is motivated to deliver quality care at a lower cost, which benefits the patient, the payer, and the provider.
